Statement of Social Security Number

In September 2001, the Judicial Conference of the United States adopted a nationwide policy to protect the privacy of certain personal data included in documents filed with the court.  

You are required to submit a Statement about your social security numbers to the court as a part of your bankruptcy filing. However, your full social security number will be provided only to those persons and entities identified on your creditor matrix, together with the United States Trustee (a unit of the Department of Justice) and the trustee assigned to your case. Other parties seeking to review your case record through the United States Courts' public access system (known as PACER) will be able to view only the last four digits of your social security number.

Except for the Statement about your social security numbers, all other documents you file with the court should comply with the privacy policy, and should include only the last four digits of any social security numbers or account numbers when such information is requested.   See also Federal Rule of Bankruptcy Procedure 9037.
