Beginning February 1, 2020, the U.S. Bankruptcy Court for the District of Minnesota will require those requesting payment of unclaimed funds to submit their request using the new, nationally-developed form Application for Payment of Unclaimed Funds (Form B 1340 as modified for use in the District of Minnesota).
