Exhibit D to the petition required in all individual cases

To ensure compliance with Rule 1007(b)(3) of the Federal Rules of Bankruptcy Procedure, Exhibit D to the voluntary petition will be required in all Bankruptcy cases filed by an individual debtor effective December 1, 2008. In the past, the Clerk's office only required Exhibit D when a Certificate of Credit counseling was not provided with the initial filing. The new rule mandates the filing of Exhibit D by individual debtors. If a joint petition is filed, each spouse must complete and file a separate Exhibit D. An Order upon partial case filing will be entered when Exhibit D is not filed with the petition.
