Formatting Changes to the Local Forms

The United States Bankruptcy Court, District of Minnesota will be modifying the local forms on the court's website between November 16, 2015 and December 1, 2015. The content of the forms will remain largely unchanged, but the format of the forms will be updated and standardized (e.g. font, header, footer, revision date, fill-able), and some minor corrections made to outdated statutory, rule and other references. In addition, some forms will be available in Word format for easier editing.

The following forms will be modified to replace references to "Debtor" and "Joint Debtor" with "Debtor 1" and "Debtor 2," conforming to the verbiage of the December 1 forms:
- Amendment to Petition
- Amended Schedules A/B and C for Real Property
- Signature Declaration
- Payment Advices Cover Sheet
- Change of Address

Attorneys were previously advised of amendments to Local Forms 1007-1 (Disclosure of compensation), 1019-1 (Conversion of Case by Debtor) and 1007-3-1(7) and (13) which will take effect December 1, 2015.

Attorneys and parties should begin using the updated versions of the forms at the earliest opportunity.
