Effective March 1, 2018, the "wherefore" clause of the Application for Compensation in Chapter 13 Case (Local Form 2016-1) has been amended to state:

Wherefore, the applicant requests the Court to enter an order awarding $_________ for total compensation or reimbursement and authorizing the trustee to pay the unpaid balance to applicant as provided in the plan.

Attorneys should enter the total amount of compensation requested in the blank AND when prompted to enter the amount of compensation requested when filing the document in CM/ECF.

Attorneys using older versions of the form after March 1 will be asked to resubmit an amended application using the correct version of the form. The updated version can be found on the court's website under the Code, Rules and Forms tab, then Local Forms.
