Proofs of claim can be filed electronically on the court's website.  No login or password is required.  See File an Electronic Proof of Claim (ePOC).
 
Claims may also be filed in paper form by mailing or delivering the completed claim form to the appropriate clerk's office.  Claims may also be filed electronically through CM/ECF.  See Proof of Claim forms.
