Requests for payment of unclaimed dividends must be filed with the
appropriate divisional office of the clerk
and accompanied by the correct documentation, as more fully set forth below:
ORIGINAL CLAIMANT'S
REPRESENTATIVE:

Petition for Release of Funds from Court Registry
Account (Form P2 - CR).

A photo copy of the original claimant's current driver's license or other
government-issued identification with notation of the claimant's social security number.

A notarized Power of Attorney authorizing the representative to file a petition
on behalf of the original claimant.

Proof of identity of the representative in the form of a photo copy of the
driver's license or other government-issued identification bearing the representative's signature.
SUCCESSOR BUSINESS OF ORIGINAL
CLAIMANT:

Petition for Release of Funds from Court Registry
Account (Form P3-SB).

A photo copy of the original claimant's current driver's license or other
government-issued identification with notation of the claimant's social security number.

A notarized Power of Attorney signed by the successor business representative
authorizing the representative to file a petition on behalf of the successor business to the original
claimant.

Documentation establishing the chain of ownership from the original claimant to
the successor business.
TRANSFERRED CLAIMS:

Petition for Release of Funds from Court Registry
Account (Form P4-TC).

A photo copy of the original claimant's current driver's license or other
government-issued identification with notation of the claimant's social security number.

Proof of identity of the successor claimant in the form of a photo copy of the
successor claimant's driver's license or other government-issued identification with notation of the
successor claimant's social security or tax identification number.

Documentation evidencing the transfer of claim.
DECEDENT'S ESTATE ADMINISTRATOR,
EXECUTOR, REPRESENTATIVE:

Petition for Release of Funds from Court Registry
Account (Form P5-D).

Proof of identity of the original claimant in the form of a photo copy of a
driver's license or other government-issued identification with notation of the claimant's social security
number.

Certified copies of probate documents establishing the administrator, executor or
representative's right to act on behalf of the decedent's estate.
